Medicines optimisation is about making sure that any medication prescribed is the right choice for the individual patient and helps them to get the most benefit from taking their medicine.
As the population ages and life expectancy increases, more people are living with several long-term conditions that are being managed with an increasing number of medicines that need to be managed safely.
Medication safety is very important. A report by the Department of Health found that 5% to 8% of unplanned hospital admissions are because of problems with their medication. We need to make sure you are given the most suitable medications for your health condition with the least chance of getting preventable medicines-related problems, such as side effects.
There are two standards within the Medicines Optimisation domain. The aim is to reduce the variation between practices so, no matter which GP practice you go to in Salford, you know your GP is doing things in the best way.
