  • Your practice will arrange one hour long 'after death analysis' meetings four times a year to look at one death of a patient and what lessons were learnt or what could have been done differently
  • As a minimum, one GP and one other member of the practice should attend the meeting. District nurses and Macmillan nurses should be told about the meeting in advance and attend if they can
